If you are unable to attend a session, please ensure you cancel at least 24 hours in advance to avoid being charged the full rate of the session. Payments for group sessions are made in advance, or you can opt for two separate payments. Remember, group spots are reserved and will not be reimbursed if the client cannot attend.
I am in network with Aetna, Anthem, BCBS, UMR, United, and Tricare Insurance through a third party group practice and services will be provided through that practice. If you would like to submit an insurance claim independently, I can provide a “superbill” for each session to assist with this process. Please check with your insurance provider for more details on self-submissions. Please note that I am unable to provide services to individuals with Colorado Medicaid insurance.
All records requests can be sent to sara@brightsparktherapy.net. If you were treated by me under a different company you can contact that agency or facility directly, or I can direct you to the appropriate contact.

NO SURPRISES ACT - You have the right to receive a Good Faith Estimate of what your counseling prices may be, including session pricing for the services you may require. If you would like a Good Faith Estimate of the counseling prices for your treatment, one will be provided for you.
